Under the government vision for the NHS, the Choice Frameworks tries to make the slogan “No decision about me, without me”, become a reality. The 2014-15 Choice-Framework outlines your rights from April 2014.
New Patient Registration
New patients may apply at reception and complete a new patient registration form. When applying please bring your medical card if you have one.
If you wish to complete the registration form before you come along to the health centre, please download the form – GMS1 below, complete it and bring a copy with you to reception to register.
Did you know that your local pharmacy offers appointments for the treatment of minor ailments, if you have a minor ailment you can get an instant appointment by asking to see the pharmacist. The pharmacist will refer you on the your GP, walk-in centre or A&E if your condition requires this. If you qualify for free prescriptions the medication will be provided free of charge, where this is not the case you will be charged the cost of the medication or the normal prescription charge, which ever is less. Ask the pharmacy or Blackwood reception for details of the service. Some pharmacy offer extended opening hours.
To make an appointment at the practice, you can telephone the surgery or enquire at reception.
The practice participates in an advanced access scheme, which allows advance bookings to be made as well as same day appointments. You may also request to see a doctor of your preference.
Please arrive at the practice for your appointment at least 5 minutes early.
Extended Evening Surgery
Unfortunately we are unable to take part in this scheme therefore our last late night surgery will be Monday 25th September 2017.
The health centre is closed at weekends and on bank holidays.
Booking Appointments Online
To use this method you must first register at the practice for the service. To do this visit the reception taking with you some Photo ID, passport, driving license, etc. You will then be issued with a User ID and Password to use the service. Use the link below to book appointments Online. If we have GPs on annual leave or sickness due to limited availability this will not be accessible.
You can also access the service using an App on your Android or Apple smart phone or tablet computer, to use this method you must also register at the practice as above. Next you need to download the app using the link below.
This practice operates a policy of zero tolerance on violence or aggression to staff. Patients exhibiting this behaviour will be removed from the practice list and allocated to a specially designated practice via Walsall PCT. Tel no 01922720255.
Confidentiality and access to patient to patient information: All staff have access to patient information and the patients rights under the Freedom of Information Act allows patients to access such information. For further details ask at reception.
On-Line Repeat Prescriptions
Following a request from our Patient Representation Group, the practice has now introduced an on-line repeat prescription ordering facility.
To use this, please markup your existing repeat prescription form with your requirements and then scan the form as a pdf and email it to the practice at the address below. For those without access to a scanner, please download and complete the the form below, then email the form to the practice at the address below.
Please note: Repeat prescription will held until the repeat is due, if you make your request early.

A more efficient method of obtaining your repeat prescription is to use the Repeat Prescription Service offered by many Chemists and also operate a medication delivery service for patients who register with them.
We are now live for Electronic Prescribing where your prescription can be sent electronically to a Chemist of your Choice – this does exclude specific drugs e.g. Controlled Drugs.
